If you are not yet a named user of the Center —
Use this form to have your name added to the list of group members associated with a Bloomington User Number (BUN)
If you are a named user on an account —
Use this form to have your name moved from the account you are on now to a new one
If your name or e-mail address is misspelled in our records or otherwise incorrect —

Obtaining a BUN
if you are a group leader and do not yet have an account.

Enter the first and last name of your group leader or account-holder.

Enter your e-mail address (one only, please).
Submit the form. All fields are required for submission.
